Foundation resources for you
Ascension Wisconsin Foundation has numerous resources available to all associates of Ascension Wisconsin. Please use the below links to navigate to the proper documentation and request forms. Both a completed funding request along with documented Foundation approval is required prior to accruing any expenses.
Funding requests should focus on the areas of healthy equity and access, systems and innovation and/or capacity building. Documented approval from your hospital president and/or service line leader is required to submit this form.
This request form is for all patient assistance requests and are to be completed by a Social Worker, NOT a patient.
This form is for new associate certification opportunities that promote clinical excellence and advanced levels of care. This form may also be used for educational requests.
This form is for on-site education opportunities that support associate professional development opportunities that enhance expertise and levels of care.
This form is for requests of up to $5,000 that encourage creativity, problem solving and innovative solutions within the workplace. Click here to learn more.
This form is to request a foundation fund transfer for approved requests and/or allocations.
When an associate needs financial assistance, there are resources available to help.
A final report is required for all approved allocations within 30 days of final fund transfer.
